Your Visit - New & Returning Patients

We welcome you as a new patient and appreciate that you have chosen Comprehensive Pediatrics to provider care to your children.

New Patients - Enroll and Transfer Your Records

To assist you with the enrollment process, please download, print and complete the forms for The New Patient Encounter. This will facilitate the enrollment process to the practice and allow you to complete required information in the comfort and privacy of your home. Rest assured that if you are unable to download and complete the form, our patient services representative will assist you with these forms in our office.

Returning Patients / Follow Up Patients/ Change of Information / Insurance Updates

If you are a patient of the practice and any information has changed since your last visit, you may wish to download and complete the change of information/update form. Completing this prior to your visit will save you time at check in.

Insurance

Patients are expected to have the proper insurance information in order prior to the visit. This includes listing the office as your PCP. Patients who fail to present this information will be asked to pay in full or to reschedule the visit. We are providers for the following insurance companies:

Co-Pays

Although we maintain a copy of your insurance card in your chart, we ask that you bring your insurance card with you to each visit. We ask that all co-payments please be paid on the day of service.
